I line up my input shaft, I look in the alignment hole on the bottom and see that it's off center. I have tried twenty times to get the centering hole centered AND the input shaft marks aligned with no luck. Should I forget about the alignment hole and just make sure the input shaft is aligned per my marks?

Line it back up per your marks. The centering hole/pin is for when you put it on the alignment machine. Then you center the indent in the hole, insert the set screw into the hole and then set the toe with the tie rods. It could be that at some point your toe was set without using the centering pin.--Dave
