There are plenty of good shops, but to address your item b), all have some disgruntled customers. It is pretty much impossible not to, especially when you consider that owners of cars like Porsches tend to want to feel a little more special then the typical Nissan owner.
For real personal service, consider Matt de Maria. and Roger Bratter in Gaithersburg has a very small shop (AutoTherapy). in VA, Jose Herceg probably has the most personal shop (Autobahn, in Fairfax). Intersport, IMA, ASG, Taylor Chapman, etc. are all a little bit bigger.
As far as costs go, I doubt any of them are less than $135/ hr these days.
